Birmingham Hip resurfacing http://www.hipsurgery.in Introduction Hip resurfacing arthroplasty is an innovation in hip surgery. In this bone of the head and neck portion of the head of the femur is spared from being sacrificed. The upper end of the bone is just sculpted to receive a hemispherical cap or covering of cobalt chrome. The lining of the cup in the pelvis is similarly lined with a metal cup. Research is at present in progress to make hip resurfacing implants from new materials like ceramic and Oxinium. The Birmingham Hip resurfacing prosthesis is FDA approved since 2006 in the US but has been available in India since earlier. History Hip resurfacing surgery has been rejuvenated in the last decade. Traditionally total hip replacement was the established procedure for hip arthritis in the elderly. As its popularity and reach grew, younger patients started seeking a surgical solution to their hip arthritis. The established results of THR couldn’t be applied to this emerging group of young patients mainly from the developing world countries. When hip resurfacing become more popular, middle aged patients from the west started clamoring for this procedure as it offered them a great deal of freedom of movements. It is at present recommended in patients upto the age of 65 according to the NICE study. Hip resurfacing prosthesis Proxima hip replacement Another form of a conservative hip replacement is the Proxima hip which is a Mini hip or stem less hip. The diseases leading to hip arthritis in the young are 1) Early onset of Primary hip arthritis 2) Slipped capital femoral epiphysis 3) Arthritis secondary to trauma 4) Congenital dysplasia of the hip Patients with conditions like Perthes disease, rheumatoid arthritis, ankylosing spondylitis are not ideal candidates and may need another bone saving procedure like the Proxima hip replacement. Avascular necrosis is another condition where the advisability of hip resurfacing has undergone a sea change recently. Leading experts like Mr. Derek McMinn have given up doing hip resurfacing for avascular. In the light of emerging evidence, hip resurfacing is not at advisable in avascular necrosis or osteonecrosis. The main advantage is that it buys time for the young patient afflicted with hip arthritis and improves his/her quality of life. 2. Risk of dislocation is minimal 3. Full range of movements is possible with patients being able to squat on the ground. Caution with hip resurfacing Avascular necrosis is a disease with a case ratio of 1 in 20,000 of the population. Hip resurfacing was advised for this group of young patients. However recent evidence shows that after a hip resurfacing, avascular necrosis always occurs under the femoral component. This iatrogenic avascular necrosis may add to the problems of the pre AVN patient and this may result in a high failure rate. Technically also, AVN changes are quite advanced in the bone and hence a successful outcome may not always be possible. Patients with AVN are advised to seek better options like a Proxima or Silent Hip replacement.
