In August of 2000, Medicare had implemented a brand-new, very sophisticated payment system for all hospital outpatient service reimbursement – and in less than a year, it was obvious that something wasn’t working! The procedure codes were not appearing on the claim forms – and Medicare knew it was not paying the hospitals correctly but could do nothing but watch most of them try to “figure it out.” In the ensuing years, various coding and compliance issues have cropped up where utilization of Certified RN-Coders was key to helping emergency departments, hospital systems, and ambulatory surgery systems develop better operational systems to ensure and all orders and all services were correct, properly documented, then coded in “real time” – up close “where the action is.” Huge health plans started sending their review nurses in for training – and CRN-Cs assisted in settling large arbitration claims against Health Net, Blue Cross of California and PacificCare. Today, Certified RN-Coders are trained by web-casting, tested only by proctored exam, and receive their work@home via the Internet – directly into their own email account. “We’ve come a long way, Baby,” certainly seems an understatement!
